Attendance
Students are required to be at school every day when school is in session. Parents/ Guardians should complete this Attendance Form to report your child’s absence, lateness or early dismissal.
The Attendance Form should only be completed by a parent/ guardian using a verified parent email address already on file. Students should not use this form to self-report their lateness or absence.
Report an Absence
If a student must miss school, the parent/ guardian must report the absence by completing the Attendance Form by 9:30 AM. A doctor’s office note is required after three days of consecutive absences.
Report a Lateness
Please complete the Attendance Form if your child will be arriving more than 30 minutes late. A doctor’s office note is required upon late arrival after a medical appointment.
Report Early Dismissal
We encourage parents to make medical or dental appointments after 3:00 PM. However, if your child needs to leave school early, please complete the Attendance Form before 9:30 AM that day. Alternatively, your child may bring a note to the Main Office before first period class. Main Office Staff will prepare a pass to be given to your child at the time of departure.
Common Questions
If you are unable to access this form, please email 02M418_attendance@groups.schools.nyc. Include the student's name, grade, date of absence/ lateness, reason and a phone number to contact you in case the Attendance Team needs to follow up.
2. I received a message from NYC Public Schools about my child’s absence.
Whether or not the school has been notified, parents receive an automated message regarding their child's absence from NYC Public Schools. It will be sent to the contact information listed in their NYC Schools Account. You can ignore this message if you have already notified us.
3. I am returning to school after an absence.
Upon returning to school, students are required to bring a a medical note or note from their parent/ guardian to explain the absence. Students should have the note signed by all of their teachers before submitting it to the Main Office. Otherwise, the absence will be marked as unexcused. This is to ensure the student has a conversation with each teacher about missed assignments.
A handwritten note is acceptable or please feel free to use our Absence Excuse Form.
