• First Day of School Checklist

  Parents/ Guardians:

  1. Help MHS secure federal funding by completing the Family Income Inquiry Form.

  • Parents can complete one Family Income Inquiry Form for all their children attending any NYC Public School.
  • Breakfast and lunch is currently provided free for all students and is served in the cafeteria every school day.
  • See the NYC Public Schools School Meals website for menus and more information.

  2. Complete and electronically sign the PARENTAL RELEASE FORM and STUDENT ACTIVITY & TECHNOLOGY FORM. A secure link will be sent to your Parent/ Guardian email on school record so please look out for it.

  3. Have your child bring in the Student Activity & Technology Fee, cash or check in the amount of $60 made out to "Millennium Tribeca High School PA" or pay by Zelle to the PA at:

  • Zelle: Send to pa.treasurer@millenniumhs.org

  Remember to write the student name/grade and state activity fee so that it will be properly allocated.

  *Fees brought to school will be collected during Advisory. 

  Students:

  1. Review and discuss with your parent(s)/ guardian(s) the NYC Public Schools Discipline Code and Bill of Student Rights and Responsibilities.

  2. Obtain school supplies. 

  3. BRING LUNCH (on the first day of school, students will stay inside for lunch in the cafe or lunch from home). NO OUT LUNCH until the Parental Consent Form is completed.

  4. Bring: pen/ pencil, notebook, Student Activity & Technology Fee to submit in Advisory, your MHS ID. New students will get their ID on the first day of school.

  5. AP Students only: Bring your AP homework on the first day of school!

  6. Get a good night's rest. We are excited to see you on Thursday, September 10th!
