  Please complete the Family Inquiry Income Form ONLINE.

  1. We ask all families to complete the Family Income Inquiry Form  so that MHS will receive federal funding to support our vital school programs. 

  • Parents can complete one Family Income Inquiry Form for all their children attending any NYC Pubic School.
  • Breakfast and lunch is currently provided free for all students and is served in the cafeteria every school day.
  • See the NYC Public Schools School Meals website for menus and more information.

  Please review, sign and print the PARENTAL RELEASE FORM, and STUDENT ACTIVITY FORM & FEE and SUBMIT AS SOON AS POSSIBLE.
